INTENSIVE COMMUNITY SUPPORT SERVICE

ERE (Emergency Room Enhancement), also referred to as Hospital Diversion, is an intensive Community Support service affiliated with the Kansas City Metro Community Mental Health Centers and funded by the Department of Mental Health. It targets individuals who are high utilizers of emergency services (ERs, hospital, law enforcement, EMS, and other emergent services).
ReDiscover’s Hospital Diversion Program works with referred clients to:
ReDiscover leads this initiative. Collaboration involves the Kansas City area Community Mental Health Centers, area hospitals, ancillary providers, policymakers and funders including the Missouri Department of Mental Health, and several county funders.

Hospital or other behavioral health/medical professional identifies the client and submits the referral to the Hospital Diversion referral fax line and/or email address
Once referral is received, an outreach Community Support Specialist connects with the client to complete the screening process and determine program eligibility and needs.
Upon program entrance, an outreach Community Support Specialist will begin providing services to address immediate needs. This includes assistance with vital documents (birth certificate, social security card, identification), applying for Medicaid and disability (if necessary), shelter assistance, community resources, and connecting the client to longer term Community Support services and other behavioral health/physical health services.

Who We Serve:
ReDiscover’s Hospital Diversion Program serves Missouri residents who are uninsured or under-insured who have had 3 or more emergency room visits in a 90-day period.
