ABOUT US
ReDiscover’s Psychiatric Rehabilitation Services empower adults (18+) living with mental illness to achieve personal well-being and improve their daily lives.
Our comprehensive support includes case management, medication management, and counseling for individuals facing serious mental health conditions such as major depression, bipolar disorder, and schizophrenia. We also assist with chronic medical conditions, ensuring clients receive the care they need.
Enrolled clients also have access to our Healthcare Home Program, where they can enhance their physical health through exercise groups and wellness coaching. Additionally, we work to connect individuals with essential community resources for clothing, housing, transportation, and food.
We Provide:
- Medication management
- Case management
- Home visits
- Individual and group counseling
- Employment services
- Life skills education
- Referrals to additional services, as needed.
CPRP Clients also have access to ReDiscover’s Healthcare Home Program that helps individuals and families with their physical health efforts.
Program Features:
- Art therapy
- Health club
- Nutrition education
- Limited van transportation
- Semi-independent living opportunities
Who We Serve:
ReDiscover’s CPRP serves Missouri residents and accepts Medicaid, Medicare, private insurance, and self-pay clients. ReDiscover has a sliding fee scale for those who need financial assistance.
