AN ALTERNATIVE TO MENTAL HEALTH HOSPITALIZATION
ABOUT US
ReDiscover’s Thrive Center offers medication stabilization and residential hospital diversion to support individuals with mental health disorders, life crises, houselessness, and substance use disorder. We offer clinical treatment and follow up services post-discharge to ensure a smooth transition back into the community and ongoing support for mental health and housing needs.
We work closely with clients to form a treatment plan with their specific goals and needs in mind. We provide connection to community services, affordable housing, and address any medical care issues that may be part of the crisis situation.
Program Services:
- Community Support services
- Individual and group therapy
- Medication management
- Community placement in affordable housing in the least restrictive environment with meals provided
- Connection to ongoing community mental health services
- Management of any presenting medical needs
- Follow-up care for post discharge with intensive Community Support services
Referrals from Thrive may only come from ReDiscover’s Kansas City Assessment & Triage Center and the Department of Mental Health.
We help clients stabilize by providing assistance with legal issues, housing barriers, and getting access to missing documentation, as well as any additional goals clients have set for themselves.
Eligibility:
- Adults ages 18 and older
- Mental health and/or substance use crisis
- In need of medication stabilization
- No major medical needs.
- Able to self ambulate
