ReDiscover’s Transitions Program

AN OPIOD TREATMENT CLINIC

ABOUT US

ReDiscover’s Transitions Program is an Opioid Treatment Program (OTP) for adults 18 and older struggling with an opiate addiction. Transitions uses Medication for Opioid Use Disorder program (MOUD), such as Methadone and Suboxone or Buprenorphine, paired with individual and group counseling to help ensure a strong and lasting recovery. Case management services and Peer Support Specialists are also available to assist clients in identifying and obtaining resources in their communities.

EACH PERSON ADMITTED INTO TRANSITIONS WILL RECEIVE:

  • a psychosocial assessment
  • psychiatric evaluation
  • medication management services
  • individual therapy
  • group counseling and group therapy
  • community support specialist assistance
  • certified peer support specialist
  • nursing support
  • individual goals and treatment plans

We offer an additional MOUD Clinic through our Treatment Options Program.

For extra security, our front doors will lock every weekday at 11:00AM and 3:00PM. Please make arrangements with your counselor or Community Support Specialist to be let in if you come between 11:00AM and 3:00PM.

Transitions is located at 1000 E 24thStreet, Kansas City, MO 64108. The entrance is on the North end of the building on the intersection of 22nd and Campbell. When you arrive, give your name to security staff, and let them know you are arriving for an intake appointment. Have a seat in the reception area and the appropriate staff member will meet with you to begin the intake process. During this process, you will have the opportunity to meet with a counselor, diagnostician, medical staff, and case manager.

Who Qualifies?

  • Adults (ages 18 and up)
  • With an Opioid use diagnosis.
