INTENSIVE SUPPORTIVE HOUSING FOR ADULTS WITH SEVERE AND PERSISTENT MENTAL ILLNESS

ReDiscover’s Cluster Apartment Program (CAP) provides supportive housing for adults as an alternative to long-term psychiatric hospitalization. CAP staff provide intensive wrap-around support and medically necessary on-site services for individuals who have a history of failure in community settings or are an ongoing risk of harm to themselves or others. Treatment provided may include medical, dental and mental health care, vocational activities, help promoting and fostering engagement in social and recreational activities, and assisting with nutrition management and harm reduction. The goal is to house the ‘unhousable’ while teaching the necessary skills for independence to sustain their housing and improve their quality of life.


CAP serves any clients that meets CPRP guidelines, specifically those with limited functional skills and those who have never lived independently, or have failed due to repeated mental health related issues and therefore benefit from wrap around intensive support.
