COMPREHENSIVE CARE MANAGEMENT
ABOUT US
Healthcare Home (HCH) helps individuals manage co-occurring physical and mental health conditions. As a benefit of MO HealthNet, HCH is free for participants in ReDiscover’s Comprehensive Psychiatric Rehabilitation Programs (CPRP). HCH collaborates with local primary care offices to ensure physical health needs are met. HCH staff offer health screenings, preventative education, and integrated treatment planning, supporting holistic goals for better health.
Program Features:
- Comprehensive care management
- Care coordination and health promotion
- Provides a primary care physician on-site (part-time) to assist clients before they are able to get into an outside provider
- Comprehensive transitional care, including follow-up from inpatient and other settings
- Patient and family support
- Referral to community and support services
- Access to a Nurse Practitioner or a Physician if consultation is needed.
Who is Eligible?
Any individual who is living with:
- A serious mental illness (including children and adults receiving psychiatric rehabilitation services under the Medicaid Rehabilitation Option), OR
- A mental health condition and a substance use disorder, OR
- A mental health condition or a substance use disorder, and one of the following chronic conditions or risk factors such as Diabetes, Asthma/COPD, Cardiovascular Disease, Developmental Disability, Overweight (BM >25), or Tobacco Use
Who We Serve:
ReDiscover’s Healthcare Home serves Missouri residents and accepts Medicaid, Medicare, private insurance, and self-pay clients. ReDiscover has a sliding fee scale for those who need financial assistance.
