ReDiscover’s Youth Community Psychiatric Rehabilitation Program (CPRP)

ABOUT US

ReDiscover’s Youth Community Psychiatric Rehabilitation Program offers Qualified Mental Health Professional Case Managers to specifically work with children who are struggling with mental health or behavioral issues. Our providers work with the children/families to educate and promote healthy coping skills, communication skills, expressing emotions, promoting academic success and support clients experiencing safety concerns (for example, suicidal ideation, homicidal ideation, and violence). We can meet the client in the home, school or community. Provide wrap around services with our family support providers. Our services are usually long-term until the client can accomplish their mental health or behavioral goals.

We Provide:

  • Case management
  • Family Support Provider
  • Community Program Coordination
  • School-based Services
  • Life skills education
  • Referrals to additional services, as needed.

ReDiscover also has a Community Psychiatric Rehabilitation Program for adults that operates out of both our Fearon Building and South Kansas City Office.

Program Features:

  • Art therapy
  • Health club
  • Nutrition education
  • Limited transportation support
  • Semi-independent living opportunities

Who We Serve:

ReDiscover’s Youth CPRP serves children ages 6 and older who reside or go to school in Eastern Jackson County, have active Medicaid, and have a Daily Living Activities (DLA-20©) score of 50 or below.
